Registering Your Child For School

School registration begins in the Spring prior to the start of a new school year. Registration requirements include the following:

FOR KINDERGARTEN:
  • Birth Certificate
  • Immunization
    • Polio (3 doses plus booster if given before 4th birthday)
    • DPT (4 doses plus booster if given before 4th birthday)
    • Hepatitis B (3 doses)
    • MMR (2 doses after 1st birthday)
    • Physician's Health Exam (CHDP form)

FOR 1ST AND 2ND GRADES:
  • Birth Certificate
  • Immunization
    • Polio (3 doses plus booster if given before 2nd birthday)
    • DPT (4 doses plus booster if given before 2nd birthday)
    • Hepatitis B (3 doses)
    • MMR (2 doses after 1st birthday)
    • Physician's Health Exam (1st grade only - CHDP form)

FOR 3RD THROUGH 5TH GRADES:
  • Immunization
    • Polio (3 doses plus booster if given before 2nd birthday)
    • DPT (4 doses plus booster if given before 2nd birthday)
    • MMR (1 dose after 1st birthday)

Once all documents are complete and submitted, your child will be assigned to a classroom is space is available. If more students apply during the open enrollment period than we have space to take, we will hold a public lottery to allocate all spaces available.

NO STUDENT WILL BE ALLOWED TO REGISTER WITHOUT DOCUMENTATION OF IMMUNIZATIONS. CURRENT STUDENTS NOT MEETING THESE REQUIREMENTS WILL NOT BE ALLOWED TO ATTEND SCHOOL UNTIL ALL IMMUNICATION AND REGISTRATION REQUIREMENTS HAVE BEEN MET.
